MISSED APPOINTMENT FEE (NO SHOW)

This office requires 48-hour notice if you are unable to keep your new patient or initial visit and 48-hour notice if you cannot keep your follow-up or routine visit. If you miss an appointment or fail to give sufficient notice, you will be charged $75.00 for that missed appointment. This payment is expected before any further treatment is provided, or supplements can be purchased.
